I would imagine you're losing contact with your #3PP? Try 'spearing' the ball with it during downswing. In addition, try tracing the balls target line with your right forearm through impact. I had the same problem a few years back. I think it was caused by my following some poor advice on significantly weakening my grip. It wasn't long before several hook compensations had crept into my swing - dropping the club underplane and blowing the ball out to the right chief among them.
